How to file a claim?

You can file a claim online or by mail.

To submit online, you’ll need your Claim ID from your settlement notice.

For reimbursement claims, you may need documents such as:

  • Receipts or invoices
  • Bank or credit card statements
  • Proof of fraudulent or unreimbursed charges
