How to File a Claim?

To receive payment, you must submit a claim by June 22, 2026. You can file online using your Notice ID and PIN, or by mail or email using a completed claim form.

 

If you don’t have your Notice ID or PIN:

  • Contact the Settlement Administrator
  • Provide your full name and mailing address to request it

 

Claims are subject to verification, so accuracy matters. Payments will be sent by check, and must be cashed within 180 days.
