How Much Can You Receive?
Eligible Settlement Class Members may claim one or more of the following benefits.
Ordinary Losses
Receive reimbursement of up to $150 for documented out-of-pocket expenses related to the data breach.
Eligible expenses may include:

How to File a Claim?
To receive settlement benefits:
- Confirm you are a Settlement Class Member.
- Complete the online Claim Form using the Notice ID and Confirmation Code from your settlement notice, or download and mail a paper claim form.
- Select the benefits you’re claiming.
- Include any required supporting documentation:
- Receipts or invoices for ordinary losses
- Documentation supporting extraordinary losses (such as bank statements, police reports, identity theft reports, affidavits, or correspondence with financial institutions)
