Why the Lawsuit Exists
In June 2020, an unauthorized third party gained access to personal data held by EyeMed Vision Care. This data included:
Names, contact info, dates of birth
Health insurance and government ID numbers
Medicare/Medicaid IDs, driver’s license numbers, and more
The lawsuit says EyeMed didn’t use proper security to protect this data. EyeMed denies any wrongdoing but agreed to settle for $5 million.
Who Qualifies
You’re eligible if:
You received a notification letter from EyeMed about the June 2020 data breach.

How Much Can You Get?
Eligible individuals can claim:
Up to $10,000 for documented out-of-pocket losses (e.g. fraud, professional fees, credit protection)
Up to $100 for lost time (at $25/hour for up to 4 hours)
An estimated $50 residual cash payment (this may go up or down based on claims volume)
