Current and former Omni Family Health patients and employees may qualify for up to $5,000 in reimbursements, cash payments, and two years of credit monitoring + medical shield services.

Why is there a lawsuit?
A class action lawsuit alleged that hackers accessed and then posted Omni Family Health patients’ and employees’ personal information on the dark web. According to the complaint, the August 7, 2024 data incident exposed details such as personal identifiers and medical-related information.
Omni Family Health denies any wrongdoing, but it agreed to pay $6,500,000 to resolve the claims and provide compensation and monitoring services to affected individuals.
Who qualifies?
You may be eligible to file a claim if:
You live in the U.S., and
You are a current or former patient or employee of Omni Family Health, and
Your personal information was potentially compromised in the Aug. 7, 2024 data incident.
There is also a California subclass for individuals who lived in California at any time between Aug. 7, 2024 and Jan. 5, 2026. Members of this subclass may receive an additional pro rata payment.
How much can people get?
Eligible class members can claim one or more of the following:
Out-of-Pocket Losses (Up to $5,000)
Reimbursement for documented, unreimbursed expenses related to the data breach, including:
- Fraud or identity theft losses
- Fees for attorneys, accountants, credit repair, etc.
- Credit freeze/unfreeze costs
- Credit monitoring purchased after the breach
- Miscellaneous costs like postage, notary fees, copying, mileage
Pro Rata Cash Fund Payment
A cash payment from remaining settlement funds after expenses.
Estimate: $105.56 (if 4% of class members file claims). Final amounts depend on how many valid claims are submitted.
Credit Monitoring + Medical Shield
All class members can elect two years of Medical Shield Complete by CyEx, including:
- Medical information monitoring
- One-bureau credit monitoring
- $1 million identity theft protection
California Subclass Payment
Eligible California residents may receive an additional estimated $100 (final amount depends on claims volume).
