Who Qualifies?
You are included as a Settlement Class Member if:
Your Private Information was potentially compromised in the September 2024 Data Incident, and
You were sent an individual notification letter by American Addiction Centers.

How Much Can You Get?
To receive benefits, you must submit a valid claim form by the deadline.
