Who is eligible?

You may be included if you:

  • Paid for, purchased, or were reimbursed for a metformin-containing drug
  • The drug was manufactured, distributed, or sold by a Settling Defendant
  • The purchase occurred between July 20, 2015 and June 2, 2020
  • The drug was for personal, household, or covered reimbursement use

 

You may NOT be included if you are:

  • The defendant companies or their affiliates
  • Federal, state, or certain local government entities (in some cases)
  • Pharmacy benefit managers (PBMs)
  • Fully insured health plans (in certain defined circumstances)
  • Anyone who purchased drugs for resale
  • Anyone who already opted out of the settlement
  • Judges or court staff involved in the case

How much money can you get?

The exact payment depends on your claim type and documentation.

  • Total fund: $5,550,000

 

Eligible benefits include:

  • A share of the settlement fund (amount varies)
  • Payments for both:
    • Individual consumers
    • Third-party payors (TPPs like insurers or self-funded plans)

 

Important note:

How to file a claim?

You must submit a claim by July 10, 2026.

Online (Fastest)

  • Complete the online claim form at the official settlement site
  • Submit your information electronically

 

By Mail

Download and send your completed form to the Claims Administrator.

 

For Individual Consumers:

  • Use the Consumer Claim Form

 

For Third-Party Payors (TPPs):

  • Use the TPP Claim Form
  • May need transaction data (especially for larger claims)

 

Depending on your claim type:

  • Proof of purchase or reimbursement for metformin drugs
  • Transaction data (required for many TPP claims over $100,000)
  • Supporting records for insured or reimbursed prescriptions

 

Keep documentation even if not immediately required. It may be requested later.
